Athlete Medical & Release Forms and Information
The Special Olympics Athlete Medical Form is designed to identify health concerns that are more common among people with intellectual disabilities. Please complete the Athlete Registration, Athlete Release Form, Health History section, and Physical Form. Athlete Physical Form should be completed and signed by a medical professional. The Athlete Medical Form is good for three years from the date of the medical professional’s signature OR the date of the exam. Incomplete forms will not be accepted.
All athletes and participants need to have a COVID-19 Release form on file in order to participate. This form only needs to be submitted once. All athletes and participants need to have a Communicable Disease form on file in order to participate. This form will need to be completed every three years with a new medical form.

Basketball & Swimming - January 15
Track & Field, Powerlifting, Soccer, Tennis & Cornhole - April 1
Golf - June 15
Softball, Bocce, & Flag Football - July 1
Bowling - September 15
Skiing & Snowboarding - November 15
